You mention uPnP being active in your setup - I just checked and uPnP is disabled in my setup (per default).
I can't check now but I have a vague recollection that I may have disabled uPnP on the Netgear router I mentioned too - I'm pretty sure it was enabled by default on that router but I would have disabled it (if I did) because of the potential security issues rather than for any obvious SIP related issue as I don't run anything where uPnP is usually useful.
